A scar is a mark of a wound or injury left on the skin. In fact, a scar is a natural process of healing after the skin has been cut or damaged. A scar can be formed for different reasons such as injuries, infections, inflammation of tissue, or surgeries. A scar can appear anywhere on the body and in different compositions including flat, sunken, lumpy, or colored. The appearance of a scar depends on many factors such as the location on the body and the type of skin, the type of injury, age of the person, and the direction of the wound. You may feel itching or pain which is quite normal.

What is a Scar Revision?

Scar revision, also known as scar removal surgery is a cosmetic surgery attempting to minimize the unpleasant scars resulting from injuries, burns, acne, chickenpox, or other surgeries. In fact, a scar revision doesn’t erase a scar but attempts to improve the condition or appearance of a scar anywhere on your body and blend in with the surrounding skin tone and texture. In another word, a scar revision surgery can not erase a scar completely but reduce its appearance or impact. Depending on the type of revision procedure needed, the scar revision surgery is on an outpatient basis and can be performed under local anesthesia. The duration of scar revision surgery is between 1to 3 hours. Scar revision for a tummy tuck is the most common treatment with a high rate of demand. Scar Revision Surgery Procedure

Depending on the type and degree of scarring, different treatments might be considered. All the scars don’t need surgery.

Types of Scar Revision

border

Based on the type, size, degree, and location of scarring, various treatments can be offered as scar revision surgery. In most cases, one single procedure can be performed providing a desirable result; while, in some rare cases, a combination of procedures might be applied to achieve the most desirable result. Generally, scar revision can be performed in two ways: surgical and non-surgical procedures. We have listed all the methods as below:

I. Surgical methods:

Fusiform elliptical excision is a basic surgical technique is used to repair a mature scar that is either deep or has spread beyond the margins of the original wound. In this procedure, the scar tissue will be removed along with a small margin of normal tissue, the edges of which are then delicately stitched together with fine sutures.

The surgeon will use flaps and grafts when there are significant areas of burned or scarred tissues. In flaps method, the surgeon will cut a portion of healthy skin and shift it to an adjacent area with the blood flow still intact. In grafts skin, a healthy patch of skin will be moved to a distant part of the body, severing the blood flow and requiring the growth of new blood vessels. In this method, a balloon is placed under the skin to gradually stretch it and “grow” larger pieces of skin for transplant.

The advantages of Z-plasty are:

    • It “irregularizes” a linear scar, breaking it up and making it less noticeable.
    • It can disguise a scar by repositioning it along natural skin creases.
    • It realigns the direction of a linear scar, releasing tension.
    • It can lengthen a linear or webbed scar, reducing skin contraction.

There are several variations of Z-plasty including W-plasty (being used primarily for short facial scars) and S-plasty (being used for oval contracted scars).

II. Non-surgical methods:

Laser Scar removal causes changes to the surface of the skin that allow new, healthy skin to form at the scar site.

Injectables are medications applied topically to lighten the skin.

Chemical peels penetrate the skin’s surface to soften irregularities in texture and color.

Dermabrasion is a mechanical polishing of the skin.

Recovery and POST-OPERATIVE Cares after Scar Revision

border

After scar revision surgery, you may experience Side effects like headaches, dizziness, nausea, and fatigue which are quite normal.

The inflammatory phase is known as initial recovery phase and generally takes between one to two weeks which you may experience pain, localized swelling, and wound discoloration. You should follow surgeon’s instruction carefully to avoid of infection and wound dehiscence. You should keep the wound clean and to dress it regularly with sterile gauze and breathable bandages to prevent moisture build-up. Some wounds require nothing more than a simple dressing, while others need specialized dressings and adjunctive therapies like hyperbaric oxygen to aid with healing.

Different Types of Scars

border

As we mentioned above, the type of treatment depends on the type of scars you have. We category the type of scars as follows:

    • Hypertrophic scars are Red, raised scars caused by the excessive build-up of collagen, typically within the margins of a wound
    • Keloid scars are Fibrous tissue outgrowths that are raised and brown and can extend beyond the margins of a wound
    • Surface irregularities
    • Burn scar contractures

Things to know about Scar Revision Surgery

border

Adults and children can be candidates for scar revision surgery except people who cannot stop taking anticoagulant drugs like Coumadin (warfarin) due to the risk of thrombosis. It is very important to know that taking anticoagulants during the early recovery phase can impair healing and lead to an even worse scar. It is out of question that people with hemophilia or uncontrolled diabetes can’t take scar revision surgery as well.

Like any other surgery involving incision, scar revision surgery comes with some possible risks and complications including a risk of bleeding, infection, and wound dehiscence. The greater concern is whether the procedure will lead to even worse scarring.
